1 Full-time 9 hours ago Full Job Description POSITION PURPOSE:
A Community Health Worker (CHW) is a front-line public health worker has a very good understanding of the community served.
CHWs help develop close relationships with patients, and play a critical role in reducing health disparities and managing chronic diseases.
They are critical members of health care delivery teams who may focus on the social aspects of care that support and enhance critical clinical activities such as diagnosis, treatment, or clinical procedures.
The CHW supports providers, Public Health Nurses, and the case management team through an integrated approach to care management and community outreach.

Responsibilities:
Provide support services and assists individuals to navigate the health care system.
Encourage self-sufficiency by providing education on self-care and strategies to improve health by helping them understand their health condition(s) and optimize individual, family and community outcomes.
As members of care delivery teams, CHWs can assist individuals in managing their own health by keeping medical appointments, screenings, treatment and follow up care.
A CHW must deliver health information using culturally appropriate terms and concepts.
CHWs community outreach includes traveling to individual homes or meeting with clients outside of the clinical setting.
Establish trusting relationships with patients and their families.
Accurately record and document vital signs when indicated.
Conduct home safety assessments when indicated Maintain a patient caseload to provide individual CHW services.
Actively participate in community planning and evaluation efforts aimed at improving health outcomes.
Facilitate strategies to increase resources and the expansion of specific health awareness in communities.
Establish and maintain networks and partnerships with other community health workers and organizations for the purpose of care coordination and enhancing resources.
Accurately, thoroughly, and in a timely manner documents patient interactions and care into the Electronic Health Record (EHR).
Prepare and deliver presentations in various forums.
Perform all other duties as assigned.
MINIMUM MANDATORY
Qualifications:
Education:
High School Diploma or GED equivalent.
Successful completion of a Community Health Worker training program from an accredited college or other educational institution required or must attain within two years of employment.
Experience:
Two (2) years Community Health Worker or related experience.
Certifications:
Current and valid CPR certification or must obtain within 90 days from date of hire.
CHW State certification required or must attain through grandfathering or course completion.
Mandatory Knowledge, Skills, Abilities & Other
Qualifications:
Demonstrated knowledge of cultural and social differences of family unity, religious beliefs, health-related beliefs and practices, generational differences, traditions, histories, and socioeconomics.
Knowledge of human behavior enabling incumbent to provide counseling, guidance, and health instruction in order to create and maintain a positive health care experience for both patients and their families.
Ability to use communication strategies and methods that acknowledge the dignity of cultural traditions.
Knowledge of community health worker principles including population-based services, evidence-based services, prevention, and community assessment.
Knowledge of regulatory agencies and documentation requirements.
Knowledge of Health Center policy and procedures relating to infection control.
Knowledge of proper practice and procedures performing vital signs and emergencies including the implementation of cardiopulmonary resuscitation, CPR.
Effective communication and writing skills.
Knowledge of current computer software applications, including Word processing, spreadsheets, electronic documentation, and computerized health records.
Knowledge of performance improvement and evaluation processes.
Familiarity with the mandatory reporting requirements under the Indian Child Protection Act.
PREFERRED
Qualifications:
Previous experience, preferably in a Native American population.
Bilingual skills in English and the Keres native language.
WORK ENVIRONMENT:

This position may be exposed to certain health risks that are inherent when working within a health center facility.
PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job.
While performing the duties of this job, the employee may be required to frequently stand, walk, sit, bend, twist, talk and hear.
There may be prolonged periods of sitting, keyboarding, and reading.
The employee must occasionally lift and/or move up to 50 pounds.
MENTAL DEMANDS:
There are a number of deadlines associated with this position.
The employee must also multi-task and interact with a wider variety of people on various and, at times, complicated issues.

All employees must uphold all principles of confidentiality and patient care to the fullest extent.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
